Psychologiepraktijk Shalita, based in Amstelveen and registered with the Chamber of Commerce under number 74597736, attaches importance to the protection of personal data. This privacy statement explains how Psychologiepraktijk Shalita handles information about an identified or identifiable natural person, as referred to in the General Data Protection Regulation (Avg). <\/p>\n

1. Application<\/strong><\/p>\n

This privacy statement applies to the following categories of natural persons from whom Psychology Practice Shalita processes personal data:<\/p>\n

    2. Processing of personal data<\/strong><\/p>\n

    Psychology Practice Shalita processes personal data that:<\/p>\n

    1. a data subject has provided in person (during a discussion or meeting), by telephone, or digitally (via e-mail or web forms on the website), such as contact information or other personal data;<\/li>\n
    2. be requested from other caregivers or referrers with the individual's consent;<\/li>\n
    3. <\/em>generated during a data subject's visit to Psychologiepraktijk Shalita's website, such as the IP address, the browsing behavior on the website (such as data about the first visit, previous visit and current visit, the pages viewed and the way through the website is navigated) and on which parts of it the data subject clicks;<\/li>\n
    4. For applying e-health.<\/li>\n<\/ol>\n

      3. Purposes of processing<\/strong><\/p>\n

      Psychology Practice Shalita processes personal data for the following purposes:<\/p>\n

      1. Performing a medical treatment agreement and billing for work performed;<\/li>\n
      2. maintaining contact, through invitations to meetings and information requested by a data subject;<\/li>\n
      3. improving the practice website www.praktijkshalita.nl;<\/li>\n
      4. maintaining user statistics. Website user statistics provide information about the number of visitors, length of visit, which parts of the website are viewed and click behavior. These are generic reports that cannot be traced back to individual visitors; <\/li>\n<\/ol>\n

        4. Legal Basis<\/strong><\/p>\n

        Psychology Practice Shalita processes personal data on one of the following legal bases:<\/p>\n

        1. consent of the data subject. This consent may be withdrawn at any time, without prejudice to the lawfulness of the processing based on the consent prior to the withdrawal; <\/li>\n
        2. performance of -or with a view to the conclusion of -a medical treatment agreement, including billing to third parties, such as the health insurance company, etc;<\/li>\n
        3. A legal obligation, such as the obligation to keep medical records or register the BSN;<\/li>\n
        4. A legitimate interest, such as using contact information to invite you to a meeting.<\/li>\n<\/ol>\n

          5. Processors<\/strong><\/p>\n

          Psychologiepraktijk Shalita may engage service providers (processors) to process personal data, who will process personal data only as instructed by Psychologiepraktijk Shalita. Psychology Practice Shalita concludes a processor agreement with processors that meets the requirements of the General Data Protection Regulation (Avg). <\/p>\n<\/div><\/div>\n<\/div>\n\n

          6. Sharing personal data with third parties.<\/strong><\/p>\n

          Psychology Practice Shalita shares personal data with third parties, if this is appropriate in the context of treatment (e.g. a referral) or is necessary for compliance with a legal obligation. Psychology Practice Shalita does not share personal data with third parties for commercial purposes, unless meetings are organized together with another organization. In that case, only necessary contact information will be shared. <\/p>\n

          7. Transfer outside the EEA<\/strong><\/p>\n

          In principle, Psychology Practice Shalita does not transfer personal data to countries outside the European Economic Area (EEA). Should this nevertheless be necessary, Psychology Practice Shalita will ensure that the transfer only takes place if the European Commission has indicated that the country in question offers an adequate level of protection or if there are adequate safeguards within the meaning of the General Data Protection Regulation (Avg). <\/p>\n

          8. <\/strong>Retention of data<\/strong><\/p>\n

          Psychology Practice Shalita does not retain personal data longer than necessary. In principle, Psychology Practice Shalita uses the following retention periods: <\/p>\n

          1. medical records: at least 15 years after the end of the treatment agreement (as of Jan. 1, 2020, it will be 20 years);<\/li>\n
          2. (financial) administrative data: 7 years after data capture;<\/li>\n
          3. data of employees and freelancers, other than (financial) administrative data: 5 years after leaving employment or after the end of the assignment contract, respectively;<\/li>\n
          4. applicant data: 6 months after completion of the application process;<\/li>\n
          5. visitors to the website: 5 years after the last visit to the website, unless objection is made earlier in which case destruction will be made.<\/li>\n<\/ol>\n

            9. Privacy statement changes<\/strong><\/p>\n

            Psychology Practice Shalita may change this privacy statement at any time. A current version of the privacy statement will be published on Psychology Practice Shalita's website. You should check this privacy statement periodically to ensure that you are aware of any changes. <\/p>\n

            10. <\/strong>Rights, questions and complaints<\/strong><\/p>\n

            You have the right to request Psychology Practice Shalita to inspect, rectify, delete, transfer, restrict processing and object to processing of personal data. You can contact Psychology Practice Shalita about this by sending an e-mail message to erez@praktijkshalita.nl. <\/p>\n
